Forms

The following forms are available for use in accordance with the services provided by the Office of the Anti-Discrimination Commissioner.  Completed forms can be faxed (03) 6233 5333 or emailed to antidiscrimination@justice.tas.gov.auForms can be completed in languages other than English and will be translated by this office.

Complaint form  - to lodge a complaint of discrimination and/or prohibited conduct.

Enquiry form (online) - to make an enquiry about discrimination and/or prohibited conduct under the Anti-Discrimination Act 1998 or about the functions of this office.

Incident Report Form (online) - to report incidents of discrimination, harassment and/or inciting hatred happening in the community. 

The Incident Report Form was developed in consultation with the Department of Immigration and Citizenship - Tasmanian Settlement Network to provide individuals and service providers with a mechanism to report incidents without having to lodge a formal complaint. See more information about the Incident Report Form.
